CHR Registered Home Health Care Nurse

Job Summary:
The Tribal Registered Nurse is an important part of the Community Health Representative Team. The
Registered Nurse will be clinically competent while delivering care to the Kaibab Paiute Tribal patients,
working to promote and maintain the health and welfare of Tribal members residing on or near the
reservation. Utilizing the process of assessment, planning, intervention, implementation, and evaluation
within the scope of the CHR and Diabetes contracts and grants program. Will work effectively with
patients, significant others and other health team members on proper nutrition and fitness while
minimizing the incidence of diabetes and other chronic diseases with the tribal community. One evening
shift per month is required.

Part-Time/No Benefits (20hrs/wk) $40/hr
Full-Time/Benefits after 90 days (30+hrs/wk)$35/hr

Duties & Responsibilities:
  • Clinically competent while delivering care to Kaibab Paiute Tribal patients in the community. Through consistent case management and home visits, the RN will effectively interact with patients, significant others, and other health team members.
  • Provide services through case management on a consistent basis, including screening, planning,
  • intervention, implementation, and assessment of patients for diabetes, nutrition and overall health and wellness.
  • Develop and implement nutritional principles, diet, food selection and economics. Adapt teaching plans to individuals and groups based on lifestyle and cultural preferences.
  • Work collaboratively with other members of the healthcare team in coordination of the patient’s care.
  • Conducts or participates in in-service education and consultation with professional staff, supporting personnel of own and related organizations.
  • Performs other job-related duties as assigned to maintain and enhance departmental operations.
Knowledge, Skills, and Abilities:
  • Must be self-motivated, communicate effectively, and a have great work ethic.
  • Possess a strong desire to motivate the community, organize workshops/activities and work well with groups and one on one.
  • Must be able to lift at least 50 lbs., bend, lift, climb stairs, etc.
  • Must have excellent computer skills and know how to use Microsoft Office and Adobe PDF.
Minimum Qualifications:
  • Graduate of an accredited school of nursing with current and valid Nursing License/Compact States/current BCLS.
  • Bachelor of Science in health and nutrition or field related to diabetes management.
  • Possess Food Handlers Card or receive it within 1 month of hire.
  • Must pass a fingerprint and criminal background check according to P.L. 101-630 standards.
  • Possess a valid driver’s license with a good driving record and be insurable.
